The following spells are provided here for three reasons: 1) in case you really want defined game mechanics for trans characters to get the body they desire, 2) as an expression of bemusement at the "Pregnancy" spell in the recently-released Red Book of Magic, 3) as an excuse to talk about transness in Glorantha.
Freemartin

This spell enables the target to become capable of impregnating people if they could not otherwise do so. This spell does not permit interspecies pregnancies on its own.

This spell operates in a special fashion. When cast using a single Rune Point, this spell causes the target's body to change shape in accordance with a desired vision along gendered norms. This does not alter any characteristics (eg SIZ), but does allow them to make their body more masculine, more feminine, or more androgynous in a particular fashion. They remain recognizable to people who know them.
By stacking an additional point, the following effects may be achieved (1 per additional point):
1. The target's body may be induced to alter between multiple shapes along a predetermined temporal or spatial cycle (annually, each season, with the phases of the Red Moon, daily, etc). The speed of the change may be selected by them as well. There is no inherent limit to the number of different shapes, but there are obviously practical ones.
2. The target's body may be induced to alter between multiple shapes in accordance with the target's desires or mood, again following the same rules as the previous effect. This may be done according to the player's wishes, or the player may, at their own choice, set up conditions based on Runes or Passions and check them every so often for the character's transition.
3. The target's body may be reshaped in a more dramatic fashion. This does not alter any characteristics, but it does allow, for example, the character to reshape how they appear as if their SIZ was different than how it actually is.
4. The target's body may be reshaped in a way that disguises their appearance. Anyone who has a Passion which incorporates the target on a personal level (Loyalty to the same clan or tribe does not count as on a personal level) has that Passion tested and on a success recognizes the person. Anyone without such a Passion, but with reasons to potentially recognize said person, may roll CHA vs. CHA to determine whether they do so.
There is no benefit to stacking more than four additional Rune points on the spell. This spell does not convey fertility- the Freemartin and/or Pregnancy spells must be used for those purposes.
These spells are generally not universally known, but are often formalized into hero cults, local or regional, that are then located at particular shrines, temples, lodges, etc. Alternately, any given shaman can locate spirits that will cast these spells. They are commonly-known enough that it's not an arduous task to have them cast- outside of play, assume that it takes two weeks to have either one cast upon you. The cost is as normal for Rune magic, typically paid in service or goods rather than in cash.
The Freemartin spell does not necessarily involve a reshaping of the body- sometimes this is done, sometimes the body remains unchanged but is able to impregnate anyways, sometimes the person the spell is cast upon receives a focus that they must touch to activate the magic, sometimes they are taught a mantra or mudra or mandala to chant or perform or meditate upon to activate the magic. The particular choice is, if it is at all relevant rather than X-carded or behind a Veil, up to the player. Outside of player decisions, the variations on the spell are dependent on the particular source.
Ah, actual game mechanics, if only lightly so. If for whatever reason you actually use these spells, you should probably adjust Pregnancy to have a permanent effect, the existence of which is unclear in the Red Book of Magic's current printing. It would also be a good idea to write down the cyclical or emotive changes somewhere on an annex to the character sheet if you use those options.
There's not really any particular reason why these spells couldn't reduce characteristics- the benefits are always outweighed by the losses- but I leave that up to you and your table to deem appropriate for roleplaying purposes or not.
